The Licensed Practical Nurse is responsible for assisting the Director of Health Services and/or Registered Nurse designee with the overall implementation, delivery and coordination of Resident care and treatment.
The LPN will also assist the Director of Health Services in monitoring the medication management program.
COVID-19 Vaccine required.
Duties:
Observe the resident's health status and health care needs, monitoring identified medical conditions, clinical behavior, nutritional status, personal hygiene, the resident' s response to treatment and care, and his/her daily activity level.
Reads communication log on a daily basis and records appropriate information throughout the shift.
Document in the Medical Record/EMR and report changes to Registered Nurse for further assessment and documentation in chart.
Assign and supervise personal care responsibilities delegated to Care Associates.
Administer medications Document nursing services provided in resident's wellness file.
Link the resident to necessary health-care services outside the facility.
Serve as an advocate to assist the residents with problem resolution.
Assist the Director of Health Services with the coordination and implementation of quality assurance activities.
Education:
Graduation from an accredited School of Nursing Current NJ License as a Licensed Practical Nurse
Experience:
Minimum of 2 years full-time experience with assisted living ,hospital, long-term, environment.
